Complete the required information to submit a request to exchange non-working Pyxis CUBIE pockets .

After submission, a temporary reference number, along with the pocket exchange information will be e-mailed to the contact email address. CareFusion will review the request and within 1 business day will e-mail a confirmation orderr number indicating that the request has been processed. Once you receive the confirmation order number, please reference this number when contacting CareFusion with questions.

Shipment of replacement Pyxis CUBIE pockets:
A purchase order is required for the shipment of replacement Pyxis CUBIE pockets. Each shipment of replacement Pyxis CUBIE pockets will contain an ARS shipping label and a Return Material Authorization (RMA) form.

Return shipment of non-working Pyxis CUBIE pockets:
The RMA label must be included with the return shipment of non-working pockets. For this reason, we suggest using the same box to return all non-working Pyxis CUBIE pockets to CareFusion. Upon receipt by CareFusion, a credit will be issued for the replacement pocket invoice amount after validating that the quantity and size of returned non-working pockets matches your order for replacement pockets.
